The post holder will be responsible for leading the newly configured Speech and Language Therapy team to continue to innovate and develop the community Speech and Language Therapy service in Hounslow, alongside the service manager. The team treats people with (long-term) acquired and progressive neurological conditions including Stroke, Parkinsons Disease, MS, MND, (etc,) frail elderly and dementia patients and those with a cancer diagnosis giving rise to communication and/or swallowing problems.

Alongside the Neuro SLT team there is an ENT service for those with Voice problems.

Patients can be seen in the out-patient clinic, Nursing homes and in their own homes.

The Service has links with other multi disciplinary adult teams: -

  • Community Parkinson’s Disease Nurse Specialist
  • Integrated Community Response service (iCRS).
  • Dietetics service.
  • Colleagues in referring acute hospitals in South London
  • Community Nursing teams
  • Motor Neurone Disease (MND) Team
  • A South London Palliative Care Team


  • To lead the small Neuro speech and language therapy service, being responsible for ensuring patients are seen within set waiting times as well as developing new pathways across the borough
  • To deputise for the service manager and assist with the clinical and operational management of the Speech and Language Therapy Service.
  • To work as an autonomous clinician in the assessment, diagnosis and management of adults with communication, cognitive and/or swallowing disorders as a result of a range of acquired and progressive neurological conditions including Dementia; this will also include people with other conditions such as Chronic Obstructive Pulmonary Disease (COPD) who may have swallowing disorders.
  • To help develop and implement the Rapid Response Dysphagia Screening Service (Monday to Friday 9am – 5pm)


  • To lead the speech and language therapy team, contributing to the planning, development and delivery of an effective and efficient service.
  • To be the lead in the management of dysphagia and communication holding responsibility for own caseload and providing support and guidance to more junior staff in this area.
  • To lead the speech and language therapy team, in the evaluation and audit of practice.
  • This may lead to the implementation of specific changes to practice or contribute to service protocols/care pathways.
  • To liaise with other involved agencies and with clients and carers to provide an integrated service.


  • To be the lead clinical communication and dysphagia specialist for the team and to provide specialist speech and language services for adults with acquired or progressive communication, cognitive and/or swallowing disorders.
  • To lead the speech and language therapy team, promoting multi-disciplinary and multi-agency working and client-centred care.
  • To assess the need for alternative and augmentative communication (AAC) in liaison with specialist centres
  • Assess client understanding of treatment proposals, gain valid informed consent and have the capacity to work within legal framework with clients who lack capacity to consent to treatment.
  • To refer clients where appropriate to outside agencies on discharge and write discharge reports to ensure continuity of care.
  • To ensure equal access to professional support guidance and treatment of clients irrespective of cultural or linguistic background.
  • To provide second opinions and specialist clinical advice to other speech and language therapists at all grades with regard to clinical specialism.
  • To refer clients for further investigation or second opinion as required.
  • To refer and assess patients using video fluoroscopy at the local NHS Acute Trust
  • To formulate and deliver an individual speech and language therapy treatment programme based on an advanced knowledge of evidence based practice and treatment options.
  • To encourage and support the principles of clinical governance.
  • Work within the Trust clinical guidelines and Royal College of Speech & Language Therapists (RCSLT) guidelines and standard of practice.
  • To train and supervise carers in communication and/or feeding compensation

strategies to ensure carryover and optimum care.

  • To be active in maintaining knowledge of national and local standards, monitoring own and others quality of practice as appropriate
