This is a really exciting role working across the district nursing team, with the potential to gain experience in Adult Social Care and Mental Health. You’ll be providing care to patients over the age of 18 in their own homes, this can be a range of patients; predominantly the elderly and working within mental health and social service but this is a fast growing and developing service and the career progression is endless.

You will be expected to work autonomously but you will not be alone. The team is supported by a very friendly team of nurses, therapists and Doctors working closely together to achieve the best outcomes for our patients.

This role will involve:

Providing personal, therapeutic and social support as agreed in patients’ individual intervention plans (which are set by a health or social care professional) whilst also encouraging the patient to remain as independent as possible.

  • To assist in providing nursing care as delegated by the District Nurse within patient’s homes.
  • Assist the regional nurse in the technical aspects of nursing care e.g Dressings, glucose monitoring etc. following a period of training and when deemed competent to do so.
  • Reporting any changes in the patient’s condition / progress to the District Nurse or Team Leader
  • Working as part of the multi-disciplinary team to provide various care including personal, social and palliative care, and any other components of the community nursing service (and others as they are developed).
  • Providing dignity, respect, confidentiality and safety at all times
  • To carry out phlebotomy in clinical settings and patients own homes following full training
  • Working on a rotational basis with Community Nursing.

  • Ability to work both independently and also as part of the wider multidisciplinary team
  • Strong written and verbal communication skills
  • Excellent time management and organisational skills
  • Ability to relate to both patients and colleagues in a sensitive manner
  • Flexible approach to work with the ability to work across a seven day service.
  • Ability to deal with a range of difficult or emotional situations – always demonstrating a sensitive and diplomatic approach
  • The desire to undertake further training as appropriate
  • To have access and use of a car with ability to work across different locations
