This is a really exciting role working across Community Rehabilitation, Rapid Response and Community Nursing, with the potential to gain experience in Adult Social Care and Mental Health.

You will be expected to work autonomously but you will not be alone. The team is supported by a clinical service manager, Advanced Nurse Practitioners and Team Leader Physio and Occupational therapists.

This role will involve:

Providing personal, therapeutic and social support as agreed in patients’ individual intervention plans (which are set by a health or social care professional) whilst also encouraging the patient to remain as independent as possible.

  • Supporting the co-ordinators or Senior Rehabilitation Assistants in the daily allocation of patient visits and to comply with the allocation.
  • Reporting any changes in the patient’s condition / progress to qualified staff and relevant team member.
  • Working as part of the multi-disciplinary team to provide crisis intervention, rapid response, social and palliative care, and any other components of the rapid response service (and others as they are developed).
  • Working on a rotational basis with Community Nursing.

  • Strong written and verbal communication skills
  • Excellent time management and organisational skills
  • Ability to relate to both patients and colleagues in a sensitive manner
  • Flexible approach to work with the ability to work across a seven day service.
  • Ability to deal with a range of difficult or emotional situations – always demonstrating a sensitive and diplomatic approach
  • The desire to undertake further training as appropriate
  • To have access and use of a car with ability to work across different locations
